  • Nominations Open: Hartford Partnership for Aging Education Leadership Award - deadline September 14, 2009
    The New York Academy of Medicine's Social Work Leadership Institute is pleased to announce The Hartford Partnership Program for Aging Education (HPPAE) Leadership Awards. We are currently accepting nominations for the second annual HPPAE Leadership Awards. This award is open to all HPPAE partners.

    The Leadership Award is awarded to individuals who have demonstrated outstanding leadership in aging education in 2009. The award winners will be announced in November 2009 at the Council on Social Work Education's Annual Program Meeting in the following four categories: Outstanding Dean in Aging, Outstanding Principal Investigator Faculty/Field Director in Aging, Outstanding HPPAE Student- Graduate of an HPPAE Program and Outstanding Community Partner.   • The Promise of Care Coordination
    Models that Decrease Hospitalizations and Improve Outcomes for Medicare Beneficiaries with Chronic Illnesses

    A major new report finds that care coordination programs can reduce hospitalizations and Medicare costs and improve the quality of care for chronically ill older adults--provided the programs: promote direct engagement of teams of primary care physicians, nurses and social workers; create close communication among all providers involved in a patient's care; and empower patients and caregivers to help manage their own care. Released at the 2009 Annual Conference of the American Society on Aging and the National Council on Aging, "The Promise of Care Coordination: An Analysis of Care Coordination Models that Can Reduce Hospitalization and Expenditures Among Medicare Beneficiaries and Improve Quality of Care" can help policy-makers craft national health care reforms that will better serve older adults and their caregivers.

    The New York Academy of Medicine commissioned the report in conjunction with the National Coalition on Care Coordination (N3C). It was authored by Dr. Randall Brown of Mathematica Policy Research, Inc. Brown was the lead researcher for a recent government study of 15 care coordination programs nationwide that serve older patients. The Brown report issued Tuesday takes a deeper look at the programs that were most successful at yielding quality improvements and cost savings, to identify commonalities and determine whether there is sufficient evidence to pursue care coordination as part of a national health care reform plan.

  • Launching Region 5 and a new website for normalizing the Hartford Partnership Program for Aging Education...
    One goal of the Hartford Partnership Program for Aging Education (HPPAE) is to be the norm in aging education at all MSW programs. Normalizing this model will help the social work profession address the demands of a rapidly aging population and anticipated workforce needs by training aging-competent leaders in the field.

    In striving toward this goal, the Social Work Leadership Institute has engaged with 15 MSW programs in Texas, Arkansas, Louisiana, Oklahoma, and Mississippi, which consist of "Region 5", to launch the normalization of the HPPAE. Partnering with currently funded HPPAE programs in Texas and utilizing their expertise in implementing the program to share knowledge, information, and resources with the schools new to this unique approach to geriatric education, this is the first region in the country to begin the normalization of the HPPAE.

    To learn more about creating a HPPAE at your school, this website, www.hartfordpartnership.org, provides all the tools and resources you may need. With this wealth of knowledge and information at your fingertips, the time is now for the social work profession to train the next generation of leaders to meet the needs of America's older adults.
